About Us
Built by Someone Who Saw the Gaps Firsthand
Align Clinical Care Partners was founded by Dr. Kara James, RPh, CDP — a long-term care pharmacist who has spent over a decade watching patients with chronic conditions fall through the cracks between visits.

No patient should feel like they're managing a chronic condition alone, and no warning sign should go unnoticed just because it happened outside of clinic hours.
A Message from Our Founder
Why I Started Align
For over a decade and counting, I work in long-term care pharmacy, where I have a front-row seat to something that troubled me: patients with chronic conditions — diabetes, hypertension, COPD, heart failure — were being prescribed the right medications and given the right instructions, but the follow-up in between visits was falling through the cracks. A blood pressure reading would spike on a Tuesday and nobody would know until the next scheduled appointment, weeks later.
Nowhere did I see this more clearly than in transitions of care. A patient would move from hospital to skilled nursing, or from a facility back home, and somewhere in that handoff, critical details would get lost — a medication change here, a new diagnosis there, a follow-up appointment that never got scheduled. Years of seeing that same pattern repeat itself made it clear to me that these gaps weren't occasional lapses. They were built into the way care was structured.
That gap is what led me to start Align Clinical Care Partners. With Remote Patient Monitoring and Chronic Care Management, we stay connected to patients between visits — tracking vitals, catching warning signs early, and making sure small issues get addressed before they become emergency room visits. It's proactive care instead of reactive care, and it's built on the kind of continuity that chronic conditions actually require.

Clinical Program Director, Align Clinical Care Partners
Advocacy, because someone has to speak for the patient when the pace of care leaves them behind. Continuity, because chronic conditions don't pause between appointments. And Compassion, because it is easy, in a chart full of diagnoses, to lose the person underneath them — and I never want to.

The Thread That Holds Care Together
I've spent more than ten years in long-term care pharmacy, first as a clinical pharmacist, then supervising our pharmacist and onboarding team, and now as Clinical Pharmacist Operations Manager. What stayed with me across all of it wasn't the medications or the metrics. It was the people who got lost in the shuffle. Not because anyone stopped caring — the clinicians I worked alongside cared deeply — but because the system asks patients to carry their own story from one setting to the next, and that's a lot to ask of someone who is already carrying enough.
I saw it most clearly at the edges, in the moments between. A patient leaves the hospital for a facility, or returns home after a long stay, and in that handoff something slips. A medication that was changed. A symptom that needed watching. A question no one was there to answer. I'd meet patients weeks after a transition and realize the thread connecting their care had quietly gone slack. Nobody had dropped it on purpose. There just wasn't anyone whose job it was to hold it.
I don't approach this work from a distance. Everything I know about these gaps, I learned by standing in them — doing the work, making the calls, and walking patients and families through transitions myself. As an Age-Friendly pharmacist, I center my work on person-centered care: starting not with the diagnosis but with what actually matters to the person in front of me. That's what Align Clinical Care Partners is to me. It's the role I always wished existed in those gaps: a clinical presence that follows patients through every transition, stays with them between visits, and makes sure no one has to navigate their own care alone.
